Registration Fall 2011

Registration: This program is open to all boys and girls, ages 5 to 13 (Grades 1 through 8). Please note that we will not be accepting younger kids (based on past experience). Registration will close September 16!

To register, you will need to follow 3 steps:

Step 1: Have a valid USA Field Hockey membership click here to enroll in USA Field Hockey

The cost of this program is $35 per child, plus a valid US Field Hockey Association membership ($25 for U12, $50 for U19, valid one year). This membership is mandatory for liability insurance reasons. Also, a new online application is available, so each family is responsible to get or renew their kid(s) membership.

Please make sure you register for the program online, and take immediate action for your USA Field Hockey membership. You will need to show proof of USA Field Hockey membership by the first day of play (paper print out, or number/expiration date).

Step 2: Fill out and submit the online registration form. click here for the Youth Field Hockey Program registration form.

Step 3: Pay $35 to NCFHA to cover the program costs. Once you have your membership paper print out, or number/expiration date, please mail a copy of it with a check of $35 payable to NCFHA (for the program fee) to:

Volunteer:
As this program continues to grow, volunteer opportunities for parents are being identified. Please consider helping in some of these ways:

· Help advertise this program in local elementary and middle schools, especially in Palo Alto.
· Coordinate the “group purchase” of equipment for players at the beginning of the program.
· Provide snacks for the players at the program tournaments.
· Become an assistant coach (training provided)
